Purpose of the Baseline Questionnaire

The Baseline Questionnaire is designed to measure your starting point in four key areas—commitment, compliance, resilience, and growth—so you can track your progress throughout the 30-Day Mindful Twerking Challenge.

Completing the questionnaire at the beginning, midpoint, and end of the challenge provides a clear framework for evaluating your physical, mental, and emotional growth.


What to Expect in the Questionnaire

Participants will reflect on the following:

1. Commitment

  • On a scale of 1 to 10, how committed do you feel to completing the challenge?
  • Why did you choose that number?

2. Compliance

  • Have you followed the choreography and mindfulness exercises daily?

  • If no, describe any obstacles or challenges that have prevented consistency.

3. Resilience

  • If this is your first time completing the form:
    • Identify potential challenges you might face and plan strategies to overcome them.
  • If this is a follow-up entry:
    • Describe a challenge you faced and how you overcame it.

4. Growth

  • Reflect on any progress you’ve noticed in yourself—physically, mentally, or spiritually—since starting the challenge.
  • How do these changes align with your original goals?

5. Free Response

  • What is your personal goal for this challenge?
  • How do you feel about your progress toward that goal? Has it changed or been redefined?
